KVKK Data Subject Application Form

Under Article 11 of the Personal Data Protection Law No. 6698 and the Communiqué on the Procedures and Principles of Application to the Data Controller, you can apply to us about your personal data. Copy the form below, complete it and send it to us by one of the methods listed.
1. Where to send it

Please attach documents that support your request. If you apply on behalf of someone else, attach a power of attorney.
3. Our reply
We reply in writing or electronically within 30 days of receiving a complete application, free of charge (a fee under the Personal Data Protection Board's tariff may apply only if the reply involves an extra cost). We may ask for information to confirm your identity. If your request is refused, the reply is insufficient or we do not reply in time, you may complain to the Personal Data Protection Board within the periods in KVKK Article 14.
